Özet
Bu tez çalışması, inhaler tedavi alan 4-6 yaş grubu çocuklarda vantrilok kukla ile yapılan işlem öncesi hazırlığın korku ve anksiyete düzeyine etkisini incelemek amacıyla randomize kontrollü olarak yapılmıştır. Çalışma, Ağustos 2023 - Şubat 2024 tarihleri arasında Konya Şehir Hastanesi Çocuk Acil Servisi'ne başvuran ve inhaler tedavi alan 4-6 yaş grubu çocuklar (n=80) ile gerçekleştirilmiştir. Girişim grubundaki çocuklara inhaler tedavinin nasıl uygulanacağı işlem öncesinde vantrilok kukla eşliğinde açıklanmış ve tedavi başlatılmıştır. Veriler, "Tanıtıcı Bilgi Formu, Çocuk Anksiyete Skalası-Durumluluk (ÇAS-D) ve Çocuk Korku Ölçeği (ÇKÖ)" kullanılarak toplanmıştır. Verilerin değerlendirilmesinde karışık düzen varyans analizi (ANOVA) ve Bonferroni düzeltmesi kullanılmıştır. Girişim ve kontrol gruplarında çocukların sosyodemografik özellikleri benzerdir. Girişim grubundaki çocukların öz-bildirimine göre anksiyete ve korku puan ortalamalarında son testte anlamlı azalma görülmüştür. Son-testte girişim grubu anksiyete ve korku puan ortalamaları kontrol grubundan düşüktür (p<0,05). Ebeveynlerin değerlendirmesine göre son-testte grupların anksiyete ve korku puan ortalamalarında anlamlı farklılık saptanmamıştır. Araştırmacının değerlendirmesine göre, girişim ve kontrol gruplarında son-testte anksiyete ve korku puan ortalamaları ön teste göre istatistiksel olarak anlamlı azalma göstermiştir (p<0,05). Anksiyete ve korku düzeylerinde girişim grubunda kontrol grubundan daha hızlı azalma görülmüştür (p<0,05). Sonuç olarak vantrilok kukla ile yapılan işleme hazırlığın çocukların korku ve anksiyete düzeylerini azaltmada etkilidir. Vantrilok kukla uygulamasının 4-6 yaş çocukların inhaler tedaviye hazırlık ve uygulama süreçlerinde kullanımının yaygınlaştırılması yararlı olacaktır. This thesis study was conducted in a randomized controlled manner to examine the effect of pre-procedure preparation with a ventriloquist puppet on the level of fear and anxiety in children aged 4-6 receiving inhaler therapy. The study was conducted with children aged 4-6 (n=80) who applied to Konya City Hospital Pediatric Emergency Service between August 2023 and February 2024 and received inhaler treatment. The application of inhaler therapy was explained to the children in the intervention group with the ventriloquist puppet before the procedure and the treatment was started. Data were collected using the "Introductory Information Form, Child Anxiety Scale-State (ÇAS-D) and Child Fear Scale (CCS)". Mixed order analysis of variance (ANOVA) and Bonferroni correction were used to evaluate the data. The sociodemographic characteristics of the children in the intervention and control groups were similar. According to the self-report of the children in the intervention group, the anxiety and fear mean scores showed a significant decrease in the post-test, and the anxiety and fear mean scores were found to be lower than the control group (p<0.05). According to the parents' reports, a significant difference was not found in the post-test anxiety and fear mean scores of the groups. According to the researcher's report, the anxiety and fear mean scores in the intervention and control groups showed a significant decrease in the post-test compared to the pre-test (p <0.05). Anxiety and fear levels decreased faster in the intervention group than in the control group (p<0.05). As a result, it was determined that preparation for the procedure with the ventriloquist puppet reduced children's fear and anxiety levels. It would be beneficial to expand the use of the ventriloquist puppet in the preparation and application processes of inhaler treatment for 4-6 year old children.
