    The Effect of Dexmedetomidine on Oxidative Balance in Lung, Liver and Heart: Rat Sepsis Model

    Aim: Sepsis is a life-threatening organ dysfunction caused by a dysregulated host response to infection. Early intervention in sepsis is vital and research on the subject continues. Having sedative, analgesic, and anxiolytic properties, Dexmedetomidine (DEX) is a potent lipophilic ?2 adrenoceptor agonist with imidazole structure. In recent years, there has been an increasing number of studies on the organ protective effects of DEX. Unlike other studies, in this project proposal, it was aimed to investigate the effect of DEX applied in different periods of sepsis on the oxidative stress index in the lungs, liver and heart. Material and methods: The study was approved by the Necmettin Erbakan University Experimental Animals Ethics Committee (2020 – 017). In the study, 50 female wistar albino rats were used as experimental animals. Animals were divided into five groups: 1st group: SHAM (n:10), 2nd group: SEPSIS (n:10), 3rd group: DEX (PreDEX, n:10) applied 30 minutes before cecal ligation puncture (CLP) procedure, group 4: DEX administered 12 hours after CLP (Post12DEX, n:10), group 5: DEX administered 24 hours after CLP (Post24DEX, n:10).     Effect of Vitamin D on YKL-40: Rat Hypercholesterolemia Model

    Background and Objectives: YKL-40 is considered to be associated with cardiovascular disease (CVD). In this study, the effect of serum 25(OH) vitamin D [25(OH)VitD] differences between groups on YKL-40 was evaluated on a hypercholesterolemia rat model.Methods: Thirty-two male rats (wistar albino) were equally divided into 4 groups. The first group was the control group; the second group was high-cholesterol (H-CH) adequate vitamin D (VitD) group (H-AdeVD). The third group was the H-CH deficient VitD group (H-DefVD), and the last group was designed with the H-CH supplement VitD (H-SupVD). The feeding process consisted of 2 stages. At the first stage (5 months), the H-DefVD group was fed on VitD deficient chow, while the other groups (control, H-AdeVD, H-SupVD) were fed on standard chow. At the second stage (3 months), the H-AdeVD and the H-SupVD groups were fed on the H-CH chow, whereas the H-DefVD group was fed on the H-CH-VitD deficient chow. Moreover, the H-SupVD group was given 100 IU/kg/day VitD along with the H-CH chow. Results: Compared with the control group, interleukin-6 (IL-6), soluble intercellular adhesion molecule-1 (sICAM-1), soluble vascular cell adhesion molecule-1 (sVCAM-1), and YKL-40 values in the H-DefVD groups increased significantly (p<0.001, p<0.001, p=0.009, p=0.005; sequentially).Conclusion: It can be concluded that VitD can suppress the YKL-40, thus, it will prevent CVD development in rat. Therefore, further clinical studies related with human will reveal the effect of VitD and YKL-40 on CVD development.

    PLAZMA APELİN DÜZEYİ VE PRE-EKLAMPSİ VAKALARI ARASINDAKİ BAĞLANTININ ARAŞTIRILMASI

    Amaç Gebelikte gelişen ve multi-sistemik bir hastalık olan pre-eklampsi, yılda en az 42.000 anne ölümüne sebep olan bir patolojidir. Pre-eklampsi patofizyolojisine ilişkin teoriler, temel olarak yüzeysel plasentasyon ile ilişkili olduğu düşünülen maternal-fetal immün malperfüzyona dayanmakta ve pre-eklampsinin nedeni ise henüz tam olarak aydınlatılamamıştır. Apelin, kan basıncı regülasyonu ve vasküler fonksiyonları etkileyen, ayrıca çeşitli fizyolojik görevleri olan bir peptittir. Bu çalışmada, pre-eklampsi gelişiminde apelin-13’ün rolünün olup olmadığını değerlendirmek amacıyla normal ve erken başlangıçlı pre-eklampsili gebelerde, plazma apelin-13 seviyeleri çalışıldı. Gereç ve Yöntem Bu vaka kontrol prospektif çalışma, Kadın Hastalıkları ve Doğum Servisine başvuran gebelerin içerisinde toplam 60 gebe çalışmaya dahil edildi. Projedeki gruplar sağlıklı gebe kontrol (n:30) ve erken başlangıçlı pre-eklampsi gebe (n:30) olarak dizayn edildi. Erken başlangıçlı pre-eklampsi grubu 30. haftada pre-eklampsi tanısı konan erken başlangıçlı pre-eklampsi gebelerden oluşmakta idi. Katılımcılardan 30. hafta alınan kan örneklerinden trombosit, serum albümin, ALT, AST, üre, kreatinin ve plazma apelin-13 değerleri ölçüldü. Bulgular Sağlıklı gebe kontrollere göre, erken başlangıçlı pre-eklampsi gebelerdeki trombosit, serum albümin, seviyelerindeki azalma ve ALT, AST, üre ve kreatinin parametrelerindeki artma, istatiksel değerlendirme açısından anlamlı bulundu (p=0,011; p=0,001; p=0,006; p=0,05; p=0,001; p=0,003; sırasıyla). Plazma apelin-13 değeri sağlıklı ve erken başlangıçlı pre-eklampsi gebeler arasında karşılaştırıldığında anlamlı bir fark bulunmadı. Sonuç 30. haftadan sonra gelişen erken başlangıçlı pre-eklampsinin teşhisinde Apelin-13’ün tanısal değeri bulunmamaktadır.
